Clinicians, hospitals, case managers, EAPs, and other referral partners can call to discuss adult addiction and dual diagnosis treatment scope, current availability, continuity needs, and the ASAM-informed assessment process.

Potential referrals

  • Adults being considered for PHP, IOP, or outpatient care
  • Alcohol or drug use disorder as the primary treatment need
  • Dual diagnosis needs that affect relapse risk, functioning, safety, or engagement
  • Step-down planning from a higher level of care
  • People able to participate safely in an outpatient setting

  • Recent risk, hospital, or withdrawal-management needs
  • Current providers and continuity needs
  • Timing, transportation, and commercial PPO information
