Scope of this website notice

This notice describes the public Blue Springs Wellness website. It is separate from the Notice of Privacy Practices, which addresses protected health information and substance use disorder records.

Information this public website handles

This public website presents program information, directions, phone and email links, and a menu-based automated admissions guide. It does not receive or store applicant names, contact details, treatment information, insurance information, card images, or chat transcripts.

Secure admissions requests

The admissions page displays a protected FormHippo form in an embedded frame. Entries and card images are submitted directly to FormHippo and are not received or stored by this public website. Authorized Blue Springs personnel retrieve the submission through the protected admissions workflow. Do not put applicant information in social media or the automated guide.

Automated admissions guide

The guide uses preset, general answers and does not provide a free-text message field. Topic selections are not a clinical assessment, benefit verification, or communication with a clinician or admissions representative.

Tracking technologies and browser signals

This website does not use advertising pixels, session replay, cross-site retargeting, or analytics on contact, admissions, insurance, or confirmation experiences. The production policy must identify any cookies or analytics, categories of recipients, and how browser privacy signals are handled before those technologies are enabled.

Retention and protection

Admissions form entries and attachments are handled within Blue Springs' configured FormHippo environment and are subject to its privacy, security, retention, and access controls. No website or service can promise absolute security.
