What this level of care is designed to do.

PHP is the most structured outpatient addiction-treatment level offered at Blue Springs Wellness. It may be considered when lower-intensity SUD or co-occurring care does not provide enough frequency or support, but 24-hour inpatient or residential care is not indicated.

The actual schedule, services, duration, and transition plan are individualized. Admissions confirms current program availability and timing.

Care may include

  • Individualized assessment and treatment planning
  • SUD-focused groups and recovery skills practice
  • Individual addiction-treatment sessions
  • Craving management and relapse-prevention planning
  • Co-occurring mental health care tied to SUD goals
  • Coordination with medical or psychiatric providers when appropriate
  • Case coordination and transition planning

Who may be considered

  • Adults who need a highly structured SUD treatment day
  • People stepping down from detoxification, residential, or inpatient care when clinically appropriate
  • People whose alcohol or drug use, cravings, or relapse risk interfere substantially with daily functioning
  • People whose co-occurring mental health symptoms require frequent support within an SUD-primary plan
  • People able to participate safely while living outside the program

Assessment first

The team reviews substance use, withdrawal and relapse risk, health, functioning, readiness, recovery supports, and goals before recommending a level of care.

Individualized planning

Services and goals are organized around the person while keeping alcohol or drug use disorder as the primary treatment focus.

Ongoing review

Progress, participation, safety, and medical necessity inform continued care and any step-down recommendation.

Transition planning

Planning includes the supports and next level of care needed to continue progress after this program.

Important: PHP is not a substitute for emergency, inpatient, detoxification, or medically managed withdrawal services. Clinical assessment determines appropriateness.
